Acclaim Home Care Acclaim Professional Healthcare LLC is a full service home care agency. All of our care givers and/or nurse aides are our employees. Our affliliate, Acclaim Home Care Inc. on the other hand is a referral agency. While we are a non-medical agency, our team of experienced Geriatric Care Managers conduct initial interviews/consultations to determine caregiving requirements and to develop a JewelCode@ care plan for each of our client. Our caregivers/aides go through a strictly implemented prescreening process wherein interviews/skill sets are assessed, independent audit of licenses,work and character preferences are conducted and DOJ fingerprinting,live scans and PFC@Info Service checks are done. Our private duty services include assisting with all activities of daily living, medication reminders, meal planning and preparation, light housekeeping/laundry,companion care/respite care/med appointments and therapies. We also provide incidental transportation using our clients' or caregivers' cars.
