An In-Home Visit with You, Your Loved One and Family Members.¨We offer families the opportunity to get to know us better during a no-cost, no-obligation, in-home visit with one of our staff members. This visit can take place in a private residence, skilled-care facility, hospital or rehabilitation center. During this visit, we will further evaluate the needs of your loved one and your family, set goals for care, perform fall prevention and home safety checks and discuss costs and available funding options. In-Home Nurse Assessment Once all parties agree on an appropriate level of care and to begin home care services, one of our experienced staff nurses will visit to perform a complete assessment of the client's physical, mental and social needs. The nurse will then develop a personalized care plan, based on the assessment that will be followed by the caregiver at every visit. Caregiver Selection We want your home care experience to be successful. Key to that success is the development of a warm and trusting relationship between your loved one and his or her caregiver or caregiver team.
