ElderCare Resource Center eldercareathome.org The ElderCare Resource Center is an Alzheimer's/dementia specific geriatric care management firm. We are highly specialized and experienced in working closely with memory impaired patients, their families, caregivers, and members of their health care team. Our goal is to keep our patient's in the place they most want to beÂ…their own homeÂ… Utilizing our Integrated Care Management (ICM)® model, we coordinate, implement, and monitor all of the services necessary to improve the quality of care and the quality of life of the people we serve. Although there may be no cure for Alzheimer's disease, good planning and strong collaborative efforts by family members, caregivers, medical professionals, social management teams, and because we are highly sensitive to the range of emotions that accompany a new diagnosis and/or the progression of an existing illness, all clients are seen privately and by appointment only.
