Home Health & Hospice Care Home Health & Hospice Care (HHHC) is a not-for-profit Visiting Nurse Association providing services to the acute and chronically ill, children, newborns and the dying in the comfort of the individual's home, nursing facility or at the Community Hospice House. Home Care: Visiting nurses, physical, occupational, speech therapists, IV therapists, wound care nurses, social workers and licensed nursing assistants provide home care for patients recovering from illness, injury or hospitalization. Advanced Illness Management: This program addresses the specific needs of patients who have multiple illnesses and need clinical oversight to keep them well managed in their home avoiding multiple emergency room or hospital admissions. Transitional Care nurses coordinate comprehensive services and develop a plan with the patient and family for care as the disease takes its' course. Hospice Care: Nurses, social workers, licensed nursing assistants, spiritual care providers and volunteers caring for patients at end-of-life. Physicians and Nurse Practitioners are specially trained in hospice and palliative care providing advanced pain and symptom management. Grief support is offered to families and community members in need. Community Hospice House: A ten suite in-patient facility that serves patients who need medical intervention with pain and symptom management at end of life and cannot be managed in an alternate setting. Private Duty Services: Provide clinical licensed nursing assistants and paraprofessional homemakers /companions to help clients remain independent in their own homes. Services provided for as few as 2 hours or as long as 24 hours a day. HHHC also sponsors Foot Care and Blood Pressure Clinics throughout our service area. Caregivers Caregiver Skills lnclude: Ethics, Patient Transfers, Stress Management, Transition Issues, Grief Issues, Family Communication, Communicating with Dementia Patients Caregivers are employed, bonded and insured: : Yes Caregivers are available 24 hours a day: : Yes RN on staff : Yes Caregivers are available to assisted living facilities : Yes
