Home Well Senior Care - Virginia This agency was established by a Harvard MBA, who is passionate about the care seniors get in their home. We have assembled an excellent nursing and administrative team to deliver reliable, trustworthy, and outstanding care throughout the region. We are focused on reducing rehospitalizations by creating a happy and safe transition home. In facilities patients have round the clock care, so our program is designed to offer frequent care immediately upon returning home, and tapering care down as clients becomes more independent. This program, coupled with our Fall Prevention program, has been very successful in reducing falls and rehospitalization during the initial transition home. Fall Prevention. All our caregivers are trained at orientation, and again annually, in the tactical skills needed for body mechanics, mitigating environmental risks of falls, and addressing critical factors such as new medicines, non-compliance with discharge orders, or ongoing medical conditions. Before they go into the field, all personal assistants demonstrate competency with these required skill sets. Life Enrichments Activities Program. Our care targets the social/emotional well-being of our clients. As part of their initial assessment our clients are interviewed to discover their likes and interests, which we systemically introduce into care plans. LEAP allows our personal assistants the opportunity to truly engage with clients and improve their quality of life during the time we have the honor of working with them.
