M & M Home Care M & M Home Care is a locally company – not a franchise and our owner is a Registered Nurse. M & M Home Care provides premium in home care services with our professionally trained staff who deliver a comprehensive set of services within the patient's home. Our mission is to provide consistent, uninterrupted care with ongoing nursing oversight while building trusting relationships between the patient, their families and the M & M Home Care Team. Our Small Team approach pairs each patient with a select, consistent, and dedicated team of caregivers based on a variety of factors with the ultimate goal being continuity of care. This approach readily identifies changes in patient behavior and care staff are more likely to notice slight changes in a patient's health which, in turn, can prevent escalation of more serious health problems. Every patient is assessed by a Registered Nurse, prior to the start of care. The nurse creates a customized Plan of Care specific to the patient's needs. Once complete, the RN will then coordinate any additional service needs and will provide an orientation to the Care Team. Our offices are located in Livonia and Lansing - we serve the entire Metro Detroit area. A sample of our services include: personal care (bathing, grooming, dressing, transfers, etc.) rehab care transportation services dementia care companion care errands and homemaking hospice support medication reminders hospital discharge assistance respite care automatic medication dispensers and other technologies to complement care M & M Home Care is a team of Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Occupational Therapists, Massage Therapists, Wound Care Specialists, Certified Nurse Assistants (CNAs), and Home Health Aides. Together, we’ve been providing care to clients and programs since 2014. We are specifically trained to meet the medical needs of our patients with our comprehensive set of services and through our innovative training. Our goals are to manage the care, rehabilitation and recover of our patients. We also seek to leverage advanced technology to improve service, and provide care management services for all patients. The M & M Home Care team has strong foundation in providing top-rate health care within our patient’s homes. We make it a point that their in-home health care services they receive from us will gain their trust.
