Queens Adult Care Center Queens Adult Care Center/ALP is a modern and fully furnished 352-bed facility. It offers residents comfortable accommodations, delicious food, and an array of creative and innovative programs. Located in Elmhurst, in the New York City Borough of Queens, this facility is near two urban shopping malls, the recently expanded Queens Center, and the Queens Place Mall. It’s accessible to several nearby subway stations on Woodhaven Boulevard, Grand Avenue, Newtown and Elmhurst Avenue, as well as the local Q53, Q58, Q59, Q29, and Q60 bus lines. Queens Adult Care Center/ALP offers the New York State Assisted Living Program (ALP): This program is certified by the New York State Department of Health to provide Medicaid-funded home care services. The ALP is an alternative to nursing home that enables nursing home eligible residents to receive home health care services in the Adult Home delaying or preventing the need for admission to a nursing home. Care services are provided by an on-site licensed home care agency and a community certified home health agency (CHHA) under contract. At Queens Adult Care Center we work hard to ensure an independent living environment, where everyone can thrive, and succeed. We offer just the right amount of assistance tailored to each resident’s individual needs, enabling each person to live in the most independent and least restrictive environment. Queens Adult Care Center offers a wide spectrum of services including case management, recreational activities, assistance with self-administration of medication, and meal preparation.
