Our 24 hour care facility provides meals, social activities, housekeeping, and medical management, all in a secure environment. Assessments Each resident is unique and receives and individual assessment as part of their admission process. Assessments are documented in a Personal Care Plan which is reviewed by family members. Assessments are reviewed on a periodic basis to keep care at the appropriate level. Our specially designed Memory Care program and person-centered approach provides a safe, familiar, and supportive environment for residents living with cognitive challenges brought on by Alzheimer’s and dementia. Person-Centered Approach Understanding the stories and backgrounds of each resident enables our experienced staff to create personalized memory care plans including multi-sensory experiences and exercises that are unique to each individual. We understand the frustrations often associated with memory loss and work one-on-one with our residents to create a sense of belonging and purpose.
