Skilled Nursing Our highly skilled nurses provide the best quality care to residents requiring the following services: IV therapy, Dementia care, Wound care, Respite stays, Enteral feeding program, Catheter care, Colostomy care, Ileostomy care, Medication/Diabetic/Pain Management, Hospice and Restorative nursing. Rehabilitation Our rehabilitation team is committed to creating individualized therapy programs based on personal goals. Programs include physical therapy, occupational therapy and speech therapy. We encourage inpatient and outpatient clients to achieve their maximum potential of independence and ability. Outpatient clients receive care from the same therapists through the entire program. Restorative Nursing We seek to maximize the quality of life for our long-term care residents. Our staff develops individualized treatment plans for resident care in the areas of Ambulation, Range of motion, Activities of daily living, Positioning, Alignment, Skin integrity, Wound management, Bowel and bladder training, Rehabilitative and social dining. Dietary Program At West Anaheim Extended Care we pride ourselves in changing our meals seasonally which allows us to prepare each meal with the freshest ingredients. Each meal is prepared to meet each resident’s individual needs and choices by our skilled chefs. Our dietary program offers a full time, on staff Registered Dietitian who provides regular assessments and education to residents. We also offer a select menu offering residents several choices for meals according to physician order. Activities Program A positive and uplifting atmosphere can set the tone for a resident’s stay. We employ outgoing and friendly professional staff to create social programs and activities. We often specialize activities to match the capabilities and needs of each resident, which include outings, Art Therapy, music, games and more. Social Services Our social service team assists both residents and their families during transitional periods. We also provide support, access to community resources, manage requests and concerns, and help in care planning and discharge planning for each resident.
