Before treatment begins, our Client Services Liaison visits with you and your family to determine how our services can best meet your unique needs. And, members of our team, including your Certified Home Health Aide and a Registered Nurse, meet with your family to develop a customized plan. The plan is monitored and modified as time progresses or as your needs change. To ensure the best care is always delivered, our highly trained team of Registered Nurses and Certified Home Health Aides are required to continuously update their education. Using our innovative training modules, based on the latest studies and data from associations such as the Alzheimer’s Association, the FDA and the American Medical Association, our team is always on top of the latest best practices. Our aides are also trained in the following areas to ensure our patients remain happy and healthy at home: Provide empathetic hospice care for you and your family Understand the personal and cultural preferences of each patient Monitor for subtle changes in health status and report any changes directly to your Registered Nurse for immediate intervention Our services include but are not limited to: Live-in or hourly support from Certified Home Health Aides Assistance with day-to-day activities such as dressing, bathing, and feeding Help with simple exercises that may be a part of a physical therapy program Assistance with household chores such as laundry, pet care, meal preparation, dishes and changing of bedding Help with daily errands, such as grocery shopping, dry cleaning and filling prescriptions Accompaniment services to and from appointments or personal activities Social companionship and emotional support Plus, many more home care and hospice related services
