We pride ourselves on providing dependable, highly personalized attention and services 24 hours a day, seven days a week. When we receive a request for service, an initial complimentary consultation follows, after which our approach consists of three elements: Assessment- A Registered Nurse and a Social Worker meet with the client/family to conduct a thorough evaluation, also known as an assessment. A plan of care is then developed in collaboration with the client's physician. The plan of care specifies which services are recommended, as well their frequency. Assistance-The plan of care, tailored to meet individual needs, can include: Nursing Services Medication management, to insure medications are taken in the proper dosages Monitoring of vital signs Education for both patient and family Certified Hourly/Live-in Home Health Aide Services These trained caregivers, who are routinely supervised by a Registered Nurse, provide caring companionship and help with the following activities: Bathing, grooming and dressing Light housekeeping, including meal preparation; laundry; homemaking tasks; and errands Medication reminders Advocacy-The elderly and their families often need guidance in navigating the health care system and locating the resources they need. Our geriatric care management service, provided by a social worker or registered nurse, provides assistance by: Evaluating needs Identifying available community resources and programs Coordinating services with physicians and other professionals Assisting with long term care arrangements/placement Our clinical and professional expertise enables us to provide a wide array of services often not available from traditional home care providers.
