Home Care Assistance of Kansas City Our Care Managers are health and human services professionals with degrees in nursing, social services, economics and other disciplines as well as a specialized body of knowledge and experience in aging and healthy longevity. Care Managers are trusted advisors to our clients and their families on a wide range of health, psychological, social, functional, legal and financial issues. Care Managers perform a comprehensive assessment of our clients’ needs, develop a detailed Care Plan, help select the most appropriate caregivers and then personally train each caregiver on the client’s individualized plan and needs. A Care Manager then stays engaged in each client’s care with regular visits to review and modify the Care Plan as necessary, make certain the highest quality standards are upheld, introduce new caregivers to the client’s team, and provide consistent communication with our clients and their families. A Care Manager is the single point of contact for our clients and their families and is ultimately responsible for ensuring they are completely satisfied with Home Care Assistance. This level of care management is included with our home care services at no additional charge. Home Care Assistance offers the following home care services to seniors and disabled adults: Transferring and ambulation assistance Assistance with bathing, showering, dressing, and grooming Meal and snack preparation, adhereing to client diet restrictions Self-administered medication reminders Assistance with range of motion exercises Assistance with toileting, continence training, incontinence care Light housekeeping, laundry, and linen changing Client transportaion for errands, grocery shopping, medical appointments Companionship Status reporting to family Live-in or Hourly Care Options At Home Care Assistance, we are here to serve you and your family when you need us most. For convenience, Home Care Assistance offers two types of services; live-in and hourly care. Home Care Assistance specializes in “live-in” care for those who prefer the comfort and safety of round-the-clock monitoring and care. We offer in-home care 24 hour a day, 7 days a week. A caregiver comes to your home and stays around the clock, ready to assist with activities of daily living. Our live-in caregivers are not allowed to use the client’s personal property for their own pleasure and are not allowed to have public visitors during the duration of the stay. For more flexible care options, Home Care Assistance also offers “hourly” caregivers for those who may only need assistance during certain times of the day, night or weekends.
