First - To ensure we fully understand your individual needs a professional care manager will conduct a comprehensive care assessment. Together with your input we develop an individualized plan of care based on all areas of concern. If necessary, we will also arrange to have a Registered Nurse conduct a functional assessment that ensures all aspects of care are being addressed. When skilled medical, nursing or therapy services are recommended, we will help coordinate and arrange for those services. Second - Our skilled care coordinators work continually to place the best caregiver with your loved one taking into account their care needs and individual personality. If time permits, we are able to send out caregivers for you to meet and interview before starting care; allowing you to have the choice in who provides your care. Finally - Someone is in our office and available to you 24 hrs a day. Additionally, your care manager will stay connected and provide ongoing support by adapting to the changing needs and dynamics of your specific situation.
