Welcome to Sunset Ridge Assisted Living Sunset Ridge Assisted Living is a class CNA Community Based Residential Facility (CBRF), licensed by the State of Wisconsin, providing care to residents with age-related frailties, memory loss due to illness or dementias, and those who need care during terminal illness not requiring skilled nursing (nursing home) care or hospitalization. Professional, compassionate care staff provide assistance 24 hours per day while encouraging residents to remain active physically and mentally. Our class CNA licensing allows our residents to be ambulatory, semi-ambulatory or confined to wheelchairs. Sunset Ridge is conveniently located near the local hospital, medical care, shopping, restaurants, and theaters. Our location off Highway 26 is easily accessible from the communities of Lake Mills, Jefferson, Fort Atkinson, Watertown and Oconomowoc. Level 1 Care Private unit including housekeeping, laundry service (no dry cleaning), three meals and snacks daily, scheduled activity program, monthly wellness checks, 24 hour awake staff supervision, stand by assist with bathing, provided partial assistance with dressing and/or grooming, medication management, assistance with arrangement of local transportation, provided amenities including cable television and utilities as well as all complimentary services. Shared units may be available at an adjusted fee. Level 2 Care Level 2 care includes all care services listed in Level 1 Care with the addition of any or all of the of the following: Total assistance with all personal cares, full incontinence care and/or bathroom schedule, diabetes management, therapeutic diet, individualized exercise regime (PT support). Level 3 Care Level 3 care includes all care services listed in Level 1 & 2 Care, with the addition of any or all of the of the following: Complete assistance with all ADLs including hand feeding, mild behavioral management, catheter care, and/or mechanical lift transfer. Level 4 Care Level 4 care includes all care services listed in Level 1, 2, & 3 Care, with the addition of any or all of the of the following: Specialized behavioral care planning and management, management of end of life care services under the direction of hospice or home health agency. Available Suites and Amenities Sunset Ridge offers residents units in single, shared or suite models. Residents are encouraged to furnish their units with personal furniture, belongings and wall hangings. Each unit has a private bath and large closets.
