Vitalitas Home Care Referral Agency Vitalitas Home Care Referral Agency Whether you’re seeking a caregiver to provide home care on a part-time, full-time or 24 hour/live-in basis, every in-home care professional Vitalitas refers has been carefully screened and meets the stringent standards we’ve established for inclusion with our registry. Founded with a commitment to affordable, Consumer-Directed Care, we believe clients have a right to determine the services they need and should have the freedom to choose their in-home care providers. The highly qualified Professional Caregivers we refer average 10+ years related experience and can provide non-medical, elderly home care, as well as similar in-home care services to adult clients with an illness, injury or disability who require assistance with self-care. Currently, rates for the Professional Caregivers we refer range from $16.00 - $22.00 per hour, which includes the agency’s referral fee. However, individual Professional Caregivers’ rates vary and they negotiate fees directly with each client or responsible party. The services provided by the Professional Caregivers we refer can include: THE PROVISION OF FELLOWSHIP – engaging the client in social, physical, and mental activities, such as conversation, reading, games, and crafts, as well as, accompanying the client on walks, on errands, to appointments, or to social events. THE PROVISION OF PROTECTION – remaining present with the client in his or her home, or accompanying the client when outside the home, as well as, monitoring the client s safety and well- being. THE PROVISION OF CARE – assisting the client with Activities of Daily Living such as dressing, grooming, feeding, bathing, toileting, and transferring; and assisting with Instrumental Activities of Daily Living that enable the client to live independently at home, such as meal preparation, driving, light housework, managing finances, remembering to take medication, and arranging medical care. To ensure your needs are fully understood, we take a consultative approach to all inquiries and use a comprehensive assessment tool to document your individual needs, preferences and circumstances. Then, we only refer Professional Caregivers who match your needs and fit your budget.
